May H. Thao, BSN, RN Biography

Graduated as an RN in 2005, WPCC, received her BSN from WSSU in 2007. Previous to getting her RN she worked in the medical field for 12 years as a CMA. She has been with Burke Primary Care since 2006. She and her husband moved to North Carolina in 1998, for the climate and to be closer to her families. She is the proud mother of 3 children. She enjoys outdoor activities and enjoys spending time with her families.

Chronic Care Management services include:

Maintenance of comprehensive care plans
Assistance with access to care, while ensuring continuity of care
Provides care management, including needs assessments, coordination of preventive care services, and medication reviews
Management of transitions of care between various home and community-based providers involved in patient care
